One of the frequently asked questions is, “What do we need to bring with us to the funeral home?” As you prepare to visit the funeral home to make arrangements for your loved one, the Funeral Director will need the following information. Taking this information of the deceased with you will allow the process to precede smoother.

  • Full Name of the Deceased
  • Birthdate
  • Place of Birth
  • Date of Death
  • Place of Death
  • Social Security Number
  • DD-214 (Military Discharge Papers) (when applicable)
  • Current Address
  • Marital Status 
  • Spouse’s Maiden Name (when applicable)
  • Father’s Full Name
  • Mother’s Full Maiden Name
  • Race and/or Ethnicity
  • Highest Level of Education
  • Occupation
  • Doctor’s Name and Phone number
  • Life Insurance Policies
  • Pre-Need Papers (when applicable)
  • Depending on the date for the final disposition, the following information will be immediately needed:
  • 1 Picture (20 Pictures for Memorial Tribute)
  • Information for Obituary
  • Information and Participants for Funeral
  • Clergy Information
  • Clothing including undergarments
